Our business office hours

Monday, Tuesday, Thursday and Friday 8:30 am to 5 pm Wednesday 10:00am to 5 pm

Scheduling an Appointment

Please call our office at (480) 834-5414 during business hours to request additional information about our services or to schedule an appointment.

Physician Fees

Initial office visits start at $295. The initial visit cost includes additional education materials regarding diet and nutrition as well.

Be prepared for your first visit

Billing Policy

We will provide you with an invoice at the time services are rendered. Payment can be made by cash, check, money orders, traveler’s checks, MasterCard, Visa, Discover, and American Express. There are also other financing options available.

We will give you the name and phone number of independent billing agents who may be able to help you file your medical expenses from our office with your insurance carrier. However, Dr. Thomas Lodi does not participate in any Medicare or Medicaid programs. Therefore, claims will not be accepted by Medicare or any secondary insurance companies that require denials from Medicare first.

Cancellations

Our staff members’ time is valuable, so please honor them by giving at least 24 hours notice for all cancellations.

Patient Considerations

In considerations of all patients, when coming to the center, please do not wear any perfume, colognes or other strongly odorous toiletries.
